Healthcare Project Manager -- NYC -- 6+ Months

Duration: 6+ Months

Description:

Duties and Responsibilities:
1. Maintain a deep understanding of the performance measures, e.g. inclusion, exclusion criteria, applicable codes, timeframe cutoffs, etc.
2. Understand partner organization's workflow related to the priority performance measures, e.g. roles, activities.
3. Identify partner gaps between current state and ideal state for measure activities and provide recommendations for improving workflows.
4. Develop work plans for executing workflow enhancements and care transition/coordination initiatives, tracking results, and assessing risk
5. In conjunction with partner and the PPS data support team, monitor partner measure performance regularly including key performance measures,
utilizing PPS dashboards or partner-generated analysis/reports
a. Elevate potential risks for under-performance to the appropriate stakeholders
6. Identify, assess, and offer recommendations for barriers to implementation and escalate risks to partner and PPS leadership as appropriate
7. Identify opportunities for partners to utilize centralized services, e.g. IT solutions/clinical best practices
a. Offer recommendations as to how partner can best integrate into workflows and describe the benefit to partners
8. Identify resources and/or resource gaps impacting performance on measures by closely collaborating with the partner and related teams
9. Monitor progress of implementation, course correct as needed, identify proxy measures, if any, set framework for tracking implementation activities
10. Contribute to outreach, process improvement, and/or any other implementation efforts impacting performance measures
11. Maintain regular communications with partner, including on-site support, site visits, regular meetings, and/or conference calls with key
stakeholders
a. Schedule meetings, document, and manage action items
12. Perform other duties as needed

Qualifications:
Minimum Education:Bachelor's degree required
Minimum Related Experience:
5 years related health care industry experience required, preferably in the areas of project management, change management, health care deliveryre-design or integration, health policy reform.
Skills:
• Highly professional individual with strong interpersonal and leadership skills
• Ability to work with minimal supervision, manage multiple deliverables, anticipate needs, and craft and implement solutions
• Knowledge of standard project management approaches
• Outstanding verbal and written communication and attention to detail
• Working knowledge and interest in advancing the health care industry
• Advanced proficiency in Microsoft Office: Word, Excel, PowerPoint, Outlook; Familiarity with MS Visio or Project a plus
• Ability to quickly learn new and utilize new databases and software programs as needed
